چکیده

Background: Dexmedetomidine is a selective α2-adrenergic receptor agonist which is known to produce sedation, analgesia, and also has sympatholytic, anaesthetic sparing and haemodynamic stabilising properties without significant respiratory depression. Its sympatholytic effect decreases mean arterial pressure and heart rate by reducing norepinephrine release and hence improves haemodynamic stability during laparoscopy Esmolol is an ultra-short-acting β1-adrenoceptor antagonist without any partial agonistic action or local anaesthetic action which is known to produce haemodynamic stability during laryngoscopy, intubation and pneumoperitoneum .In view of the above context., the present study was undertaken for comparison of dexmedetomidine and esmolol to determine their clinical efficacy in case of laparoscopic cholecystectomies. Materials and Methods: We conducted this study as an open label observer blind prospective randomized controlled study of 100 cases. Cases were divided into 2 groups of 50 each. A total of 100 adult patients were randomly selected who fulfilled the inclusion criteria. They were divided into groups and study was conducted. Results: In this study, we measured the results of each drug by its attenuation of pressure response and recovery profile. Conclusion: From our study we conclude that both dexmedetomeidine and esmolol promote satisfactory attenuation of pressure response and help in recovery profile of the patient but Esmolol is found to be superior.

Comparative evaluation of dexmedetomidine and esmolol on hemodynamic responses during laparoscopic cholecystectomy.

BACKGROUND The advent of laparoscopic surgery has benefited the patient and surgeon; however creation of pneumoperitoneum for same has bearings during the perioperative period. These effects of pneumoperitoneum are associated with significant haemodynamic changes, increasing the morbidity of the patient. AIM The present study compared the efficacy of dexmedetomidine and esmolol on hemodynamic...

The impact of intra-abdominal pressure on the stroke volume variation and plethysmographic variability index in patients undergoing laparoscopic cholecystectomy.

The purpose of the present study was to evaluate the effect of increasing intra-abdominal pressure (IAP) on stroke volume variation (SVV) and plethysmographic variability index (PVI) in patients undergoing laparoscopic cholecystectomy. PVI examined by Masimo Radical 7 pulse oximeter and SVV determined using FloTrac/Vigileo were monitored simultaneously in forty-five patients undergoing laparosc...

A Prospective Randomised Study to Evaluate the Effects of Clonidine and Fentanyl as Premedication for Intraoperative Haemodynamic Stability in Patients Undergoing Laparoscopic Cholecystectomy

Background: Laparoscopic surgeries are commonly done in patients suffering from cholelithiasis. However, laparoscopic surgeries are associated with pneumoperitoneum, increased intra-abdominal pressure and insufflation of carbon dioxide leading to altered haemodynamic stability. We conducted a prospective randomised study to evaluate the effects of Clonidine and fentanyl in premedication for int...

The Effect of Dexmedetomidine on Haemodynamic Changes, Extubation Time and Sedation during Laparoscopic Hysterectomy

Background and Aim Suppression of deleterious effect of pneumoperitoneum is a very important goal of laparoscopic surgeries. Dexmedetomidine is a highly selective α2adrenoreceptor agonist that causes centrally mediated reduction of sympathetic nervous system activity and lead to sedation and analgesia. The aim of our study was to evaluate the effect of dexmedetomidine on haemodynamic response o...

Pain management after laparoscopic cholecystectomy-a randomized prospective trial of low pressure and standard pressure pneumoperitoneum.

BACKGROUND Abdominal pain and shoulder tip pain after laparoscopic cholecystectomy are distressing for the patient. Various causes of this pain are peritoneal stretching and diaphragmatic irritation by high intra-abdominal pressure caused by pneumoperitoneum . We designed a study to compare the post operative pain after laparoscopic cholecystectomy at low pressure (7-8 mm of Hg) and standard pr...
